1-13-39 ATTEMPTED ESCAPE. McCain sawed through bars of his isolation cell, broke one window bar of toolproof steel with some implement; sawed outside window sash and escaped from the cell block onto the Island, between 3 and 4 A.M. this date. He was apprehended by Lieutenant Weinhold and Officer Rychner, and returned to cell block about 5 A.M. Report by E.J. Miller, Associate Warden. ACTION: Examined by Dr. Horwitz and then placed in Solitary Confinement on restricted diet. (All statutory goodtime forfeited because of this violation) 11,880 days
1-27-39 Removed from solitary to Isolation Cell "D" Block.

  1. CREATING DISTURBANCE IN CELL HOUSE. Was yelling, singing, and slamming his bunck against floor at 1:15 A.M. Report by Lieut Simpson.
  2. CREATING DISTURBANCE IN CELL HOUSE. This prisoner was sliding his bed back and forth, hitting the front of his cell and making very loud noise. Report by C.Y. Hurst, Jr. Officer.
ACTION: This man created a disturbance throughout the night and threatened the officers in abussive language. PLACED IN OPEN STRIPPED CELL, ON RESTRICTED DIET, ALL PRIVILEGES FORFEITED UNTIL FURTHER ORDERS. E.J. Miller, Associate Warden.
7-15-39 Removed to isolation cell, regular diet, all privileges forfeited until further orders.
11-14-39 ATTEMPTING TO PASS CONTRABAND AND REFUSING TO SURRENDER SAME. Contraband was a note, which he refused to surrender, saying he would be a 'dam fool' to give it to me. Report by R. Dennison, Sr. Officer. ACTION: To forfeit yard privileges for one week.
1-11-40 CAUSING CONFUSION IN THE CELL HOUSE. Yelled loudly and when he entered his cell he slammed the door making a very loud noise. ACTION: Placed in solitary confinement on restricted diet and forfeit all privileges until further orders.
1-16-40 Removed to isolation cell.
5-24-40 Removed to regular cell, with work assignment in Laundry.
6-8-40 ALL PRIVILEGES RESTORED.
8-18-40 INSOLENCE. This inmate was very insolent when asked to remove his belongings from cell #286 to cell #261. ACTION: Reprimand and warned.
